Modify the contact form to look like the one we already have?

I want to integrate our contact form into IO. You can see what we have now, here: https://www.ohiomobilegaming.com/cleveland-quote/
Many customers like to do this instead of an actual quote through the system. That said, it's tedious checking these separately (they come to us in an email form).
Can we modify the contact form in settings to look something like this? Custom questions etc? Secondly, can we integrate the code into our site, or do we need to send them to the IO hosted contact form?

I noticed when tweaking the contact form that adding some of the hidden fields, like eventcity, don't result in that information being sent over the contact "lead". Is that how it's supposed to work?

I'm checking on pricing for the customization, but are you intending to have all those checkboxes do anything? Perhaps append to the notes field would be all this form could easily do.
