The same question 
If you didn't want the address/company info to show you can go to Settings-->Pages/Templates. You can edit the "Quote Request" page. Here you can change the wrapper to "none" (the info is on the wrapper of your pages) If you want to keep the info you still will edit this page but you can edit the page and type the information that you want to be listed on top....please let us know if you need any help with this!
